Fresenius Liberty Cycler Recall Overview
A Class II recall was issued for the Fresenius Liberty Cycler on November 5, 2013. According to the recall notice published by the FDA, “Fluid leaking may be observed inside the pump compartment of the Liberty Cycler and may result in dialysate becoming contaminated, potentially causing peritonitis.”
This most recent recall is not the first time the FDA has taken action to ensure the safety of the Fresenius Liberty Cycler. Recalls for this device were also issued in August and February of 2013, as well as in March 2012. While the 2012 recall concerned a software issue that could lead to overfill of the machine with dialysate, both previous 2013 recalls were ordered to address a potential leak in the cassette portion of the device.
Problems with the Fresenius Liberty Cycler Software
Fresenius issued a voluntary recall for the Liberty Cycler in 2012 because problems with the device’s software were identified. According to letter Fresenius sent to patients in March and April of 2012, “Overfill/IIPV (described in the letters as “too much fluid in your abdomen”) may be caused by inadequate drainage, and may result in serious injury or death.” This action was too late for at least one patient, who died of peritonitis possibly linked to the Liberty Cycler software problem weeks before her family received the letter from Fresenius.
Problems with the Fresenius Liberty Cycler Hardware
At least one patient has also already died as a result of peritonitis that may have been caused by Liberty Cycler hardware issues. It is believed that patients may suffer peritonitis as a result of leaks in Fresenius Liberty Cycler cassettes, which may become punctured and release too much dialysate and lead to overfill, or the presence of too much fluid in a dialysis patient’s abdomen.
